Patient Identification

Some has been discussed about the patient identification:

PatientDemographics was changed so it could store more that one identifier for the patient - https://openncp.atlassian.net/browse/PT-8.

After that, Aarne alerted about the possibility of an id: root+extesion; the root could be different from the homecommunityID.

It seams like, that this subject needs to be discussed once again.
